I understand that I will participate in a free or reduced-cost telehealth consultation with Ryan Baker-Barrett, MS, BCBA, founder of Applied Behavioral Health Practice (Provider).
My provider uses a HIPAA-compliant version of Microsoft Teams or Healthie:
- The video connection may not work or it may stop working.
- The video picture may not be clear enough to be useful.
- If I am not in a private location, others may overhear info.
This consultation is for informational purposes only. It does not constitute medical or clinical advice.
I understand that Provider is not providing professional services, and no client-Provider relationship exists or is being established through this consultation. Any information discussed should not be interpreted as a substitute for advice from a licensed professional. I am solely responsible for seeking appropriate professional advice for your specific situation.
I understand that nothing in this agreement supersedes legal obligations regarding mandatory reporting of abuse or neglect as required by law.
By joining the telehealth consultation, I hereby consent to participate under the conditions described in this document.
